NAD+ precursor trial raises blood levels but misses functional endpoints
Taking an NAD+ supplement did raise the amount of the molecule in people's blood — that part works. But it did not make participants meaningfully stronger or faster over three months. A biomarker moving is not the same as a person getting healthier.
What happened
204 adults aged 60-80 received nicotinamide riboside or placebo for 12 weeks.
Blood NAD+ roughly doubled in the treated group, confirming the compound is absorbed and used.
Primary endpoints of grip strength and six-minute walk distance showed no significant difference; two inflammatory markers improved modestly.
Why it matters
This is the clearest recent example of the gap between biomarker change and functional benefit, which is the defining credibility problem in consumer longevity.
Twelve weeks may be too short to detect functional change in this population — a null result is not proof of no effect.
It reinforces the distinction between clinical NAD+ programmes held to trial standards and supplements sold on mechanism alone.
Evidence check
Optional scientific context: how much weight this finding can carry.
- 12 weeks is likely too short for functional endpoints in older adults.
- Participants were relatively healthy, limiting room for improvement.
- Secondary marker improvements were not corrected for multiple comparisons.
- Dose was at the lower end of the studied range.

Science explained
The terminology, in plain language.
- NAD+
- A molecule every cell uses to produce energy and run repair processes. Levels fall with age.
- Precursor
- A raw ingredient the body converts into the molecule it actually needs.
- Primary endpoint
- The single result a trial is designed to prove. Missing it means the trial failed, whatever else improved.
- Secondary marker
- An additional measurement. Improvements here are suggestive, not conclusive.
What would change our view?
The findings that would raise or lower this Signal.
Longer trial
A 12-month design testing whether functional change simply needs more time.
Deficient population
Benefit shown in people with measurably depleted baseline NAD+.
Repeat nulls
Further trials reproducing the biomarker-to-benefit gap.
Dosing critique
Evidence the tested dose was too low to be informative.
